Kindergarten Student Registration

Online student registration forms for the 2021-2022 school year can be completed online at https://ecollect.accelaschool.com/Amherst 

Please also call and schedule an appointment in your child’s school to complete the in-person screening on one of the dates listed below:

  • Wednesday, April 21, 2021 8 a.m. - 4:00 p.m.
  • Thursday, April 22, 2021  8 a.m. - 6:00 p.m.

When visiting our schools, please be reminded that the following Health Mitigation Protocols are in place in our buildings:

  • COVID Screening upon entering
  • Temperature checks
  • 6ft social distancing - family groups can stay together
  • Face coverings must be worn

If you do not have internet access to complete the forms linked above, please visit one of our WiFi access locations at any of our schools, Amherst Town Hall, or the Amherst or Madison Heights Libraries. If you are unable to access these locations or do not have a device, please schedule an appointment to meet with your child’s building administrator to complete these documents when you visit with your child for the in-person screening.

If you plan to register your new student at a school outside of your assigned zone, there are specific steps to take. Please register them at their “home” school, then fill out an out-of-zone request between May 1st and July 15th.   1. Prior to entering school, the following will need to be provided to the school:
  2. Proof of Residency (for example: electric, water, or cable bill)
  3. Child’s Birth Certificate
  4. Commonwealth of Virginia School Entrance Health Form (MCH 213 G) from your doctor. This form must be completed within the 12 months prior to the first day of school and include the child’s complete immunization record and well-child check (Code of Virginia § 22.1-270).
  5. Proof of required immunizations listed below:
    • Four DPT (1 DPT must be after 4th birthday)
    • Four Polio (1dose must be after 4th birthday)
    • Two MMR (1st dose must be after 1st birthday)
    • Three Hepatitis B
    • Two Varicella (Chicken Pox vaccine) or doctor’s documentation that child has had chicken pox
    • Two or Three Rotavirus 
    • Two Hepatitis A
